DUCHESNE CLINIC

After careful consideration and discussion over many months, it has been determined that Intermountain Health's continued operation of the Duchesne Clinic in Kansas City is not sustainable in the long term. Therefore, the clinic will cease operations on June 1, 2024. The difficult decision to close the clinic came after an unsuccessful attempt to become an independent FQHC and changes in the regulatory environment. The center of our decision is ultimately rooted in serving patients with the highest quality care and offering the type of services needed in the future. Numerous similar clinics are operating in the Duchesne area that we feel can better meet the healthcare needs of the population we currently serve and provide them with access to the healthcare they need. We are partnering with these organizations to ensure all of the patients at Duchesne Clinic are connected with one of them.

 We know this news is unsettling, and caregiver teams will work closely with patients to ensure they continue to have access to the healthcare they need. We are committed to a seamless patient transition to another healthcare facility. To help with the process, we have been in discussion with similar clinics in Wyandotte County, many close to Duchesne.

 A representative from Intermountain Health will contact patients by mail and phone soon with more information. In the meantime, patients are encouraged to contact the Duchesne clinic if they need immediate assistance.

Becoming a Patient at Duchesne Clinic

Complete the Steps below to become a patient at Duchesne. We currently accept patients without insurance. Complete process is provided by the Patient Care Coordinator.

Step 2

Gather as many of the following documents as possible:

  1. Photo ID.

  2. Proof of residency (utility bill or mail with your name, address on it).

  3. Proof of income

    • Paycheck stub (if applicable)

    • Last year’s taxes (if applicable)

Bring in the documents that you can locate. If you are not currently working or you did not file last year’s taxes, we can help.

FEE SCHEDULE

We use a sliding fee schedule to determine payments. Please see the link below for our current sliding fee schedule and payment options.

We use the most current Federal Poverty Guidelines to determine financial eligibility.

  • Chiropractic consultations are available in partnership with Cleveland University Kansas City

  • Physical Therapy services are available in partnership with the University of Saint Mary.

  • We also have specialty referrals available to those who qualify.
